- Photo ID and insurance card
- List of current medications
- Any prior imaging (X-rays, MRI) on CD or digital file
- Completed new patient forms (emailed in advance)
- A list of your questions and concerns

Your Consultation
- Review of your history, symptoms, and goals
- Physical examination of the affected joint
- Review of imaging if available
- Clear diagnosis and explanation of findings
- Discussion of all treatment options — surgical and non-surgical

After Your Visit
- Written summary of your diagnosis and treatment plan
- Prescription or referral if needed
- Physical therapy referral if indicated
- Follow-up imaging ordered if needed
- Clear instructions on next steps
